F : Formulary Item

This product is on the VA national formulary and may be prescribed for dispensing from a VA Pharmacy.

F-R : Formulary Restricted

This product is on the VA national formulary and has standardized restrictions such as being limited to a defined subspecialty of prescribers.

PA-F : Prior Authorization - Facility

This product is on the VA national formulary and may have prior authorization requirements that are managed at the local facility level.

PA-V : Prior Authorization - VISN

This product is on the VA national formulary and may have prior authorization requirements that are managed at the VISN (regional) level.

PA-N : Prior Authorization - National

This product is on the VA national formulary and may have prior authorization requirements that are managed at the national level.

This term refers to specific inclusion and exclusion criteria and other clinical requirements for use of a given medication.

VISN : Veterans Integrated Service Network

This term refers to each of the 18 regional networks of VA facilities.

U/E Formulary : Urgent/Emergent Formulary Item

This term refers to the status of a drug on the VA Urgent/Emergent formulary listing. Items on the Urgent/Emergent formulary may be filled at outside pharmacies in some instances under specific provisions for Community Care.
